What Is Ellansé (洢莲丝), How It Works, and Whether It’s Right for You

Ellanse Dermal Filler Treatments

There is a question that comes up often in consultations at Mediject. Not “how do I look younger?” but something quieter. Something closer to: how do I make sure I still look like myself in five years? At Mediject, we believe aesthetic medicine should preserve identity, not replace it. Treatments such as Ellansé allow us to support the structures that naturally change with age, whilst maintaining the characteristics that make a face uniquely yours.

Ellansé (洢莲丝) is a collagen biostimulator. It is not a filler in the way most people understand fillers. Unlike traditional hyaluronic acid fillers, the primary purpose of Ellansé is not simply to add volume. It provides an immediate structural effect through its gel carrier, but its true value lies in its ability to stimulate your own collagen production over time. Ellanse works with the skin’s own biology, encouraging the production of new collagen from within, gradually restoring the firmness and structural support that the face loses over time. The results are subtle, they build slowly, and they last considerably longer than most treatments in this category.

For patients who are more interested in ageing well than in looking treated, it is worth understanding properly.

What Ellansé actually does

The active component in Ellansé is polycaprolactone, or PCL, a biocompatible polymer that has been used in medical sutures for decades. When injected into the deeper layers of the skin, PCL microspheres act as a biostimulant. They prompt the skin’s own fibroblasts to produce new collagen around them, rebuilding the internal scaffolding that supports facial structure.

This is fundamentally different from how hyaluronic acid fillers work. HA fillers add volume from the outside and hold it there until the body breaks them down, typically within six to eighteen months. Ellansé does not add anything visible. It triggers a biological process that improves the skin from within. The PCL itself gradually absorbs over time, but the collagen it has stimulated remains.

Many patients notice an immediate improvement in support and structure. The regenerative benefits then continue to develop over the following months as new collagen is produced.Depending on the formulation, Ellanse can last anywhere from one to four years. Most patients describe noticing the change gradually rather than all at once. The face looks more rested. More defined. Still entirely themselves.

The four formulations

Ellansé comes in four versions, referred to as S, M, L, and E, each corresponding to a different duration of effect. S lasts around a year, M around two, L around three, and E up to four. The right choice depends on your skin, the areas being treated, and what you are trying to achieve. Your clinician will make a recommendation at consultation based on a thorough assessment rather than a default preference.

Who tends to benefit the most

Patients who respond well to Ellansé are often those noticing a gradual, cumulative change rather than one specific problem they want to fix. The jawline is slightly less defined, or the midface has softened, maybe the temples look thinner. None of it dramatic, but all of it affecting the overall impression the face gives.

It also suits patients who have had filler before and found the results too obvious, or who are simply cautious about anything that might alter how they look. When appropriately selected and carefully placed, Ellansé can provide structural support without the excessive volumisation that many patients worry about.

Increasingly, it is used as part of a collagen banking strategy, which is a proactive approach to skin ageing focused on preserving and building collagen reserves before significant loss has occurred. For patients in their thirties and early forties, this kind of early intervention can make a meaningful difference to how the skin ages over the following decade. It is not about doing more, it is about doing the right things at the right time.

What Ellanse treatment involves

When you come for a session at Mediject it typically takes between 45 minutes and an hour. Topical anaesthetic is applied beforehand, and the treatment itself is precise and measured.

Most patients experience mild swelling or tenderness for a day or two afterwards, then return to normal life immediately. Vigorous exercise, alcohol, and significant heat should be avoided for 48 hours.

The improvement that follows is gradual and 4-6 weeks in, the skin begins to feel firmer. More resilient. By the 12 week mark most patients are seeing the full picture, and the results continue developing beyond that as collagen production builds. It is not a dramatic reveal. It is a quiet, cumulative restoration.

The Mediject approach to Ellansé

The outcome with Ellansé depends heavily on the quality of the assessment before treatment and the precision with which it is delivered. Choosing the right formulation, mapping the right areas, and understanding how Ellansé fits within the broader picture of a patient’s face all require genuine clinical experience and judgement.

At Mediject, all Ellansé treatments are delivered by Dr Sheila Li or by clinicians she has personally trained. Dr Sheila is the lead UK aesthetics trainer for Ellansé and a guest lecturer on polycaprolactone-based fillers on the Aesthetic Medicine MSc at Queen Mary University of London. She has over eleven years of clinical experience and has treated more than nineteen thousand patients. Her approach to treatment is rooted in restraint and long-term thinking.

Ellansé at Mediject is never a standalone decision. It is always considered within the context of the whole face, and often alongside other treatments such as JULÄINE™ for deeper skin quality, or botulinum toxin where appropriate, introduced gradually and only when clinically right.

What Ellansé cannot do

It is worth being clear about what this treatment is not for. Ellansé is not used in the lips, tear troughs, or nose. It is also not a quick fix for immediate visible change, and it is not appropriate for patients who want instant results or a dramatic transformation. It also cannot be dissolved, unlike hyaluronic acid fillers, which is why the consultation and the clinical relationship that precedes treatment matter so much.

Why Dr. Sheila Chooses Ellansé

As one of the UK lead trainer for Ellansé, Dr Sheila has worked extensively with collagen stimulators throughout her career. Her preference for Ellansé lies in its ability to combine immediate structural support with long-term collagen regeneration. For the right patient, it offers a balance between restoration and prevention, supporting the skin’s own biology rather than simply replacing lost volume.

Frequently asked questions

Is Ellansé safe?

When performed by an appropriately trained and experienced clinician, Ellansé has a strong safety profile and has been used internationally for many years.

Like all injectable treatments, there are risks, including swelling, bruising, infection, vascular compromise, and the formation of nodules. These risks are uncommon but must be discussed as part of a thorough informed consent process.

One of the reasons consultation is so important is that Ellansé is not suitable for every patient or every area of the face.

At Mediject, patient safety always comes before treatment. Every recommendation is based on a comprehensive facial assessment, medical history review, and detailed discussion of risks, benefits, and alternatives.

How is Ellansé different from Sculptra?

Both are collagen biostimulators but they work through different mechanisms. Sculptra uses poly-L-lactic acid and is typically delivered as a course of three or more sessions. Ellansé uses polycaprolactone and often achieves results in a single session, with more predictable longevity. We look at both in detail in our guide to Ellansé vs Sculptra.

How many sessions will I need?

Many patients do well with one session. A second session may be recommended 6-12 months later where skin laxity is more pronounced. Your clinician will map out a clear and honest plan at consultation.

Is there any downtime?

Very little, with mild swelling or tenderness for a day or two. Most patients are back to their normal routine the same day.

How long before I see results?

Most patients notice an improvement in skin quality and firmness within four to six weeks, with full results visible at around the 12 week mark. The improvement continues to build beyond this point as collagen production develops.

Why is Ellansé not reversible?

Unlike hyaluronic acid fillers, which can be dissolved using an enzyme called hyaluronidase, Ellansé works by stimulating your body’s own collagen production.

Once new collagen has formed, it becomes part of your natural tissue. There is no medication that can selectively remove this collagen in the way hyaluronidase removes hyaluronic acid filler.

This is one of the reasons why practitioner experience, patient selection, and treatment planning are so important.

At Mediject, we take a conservative approach to regenerative treatments. The goal is always natural-looking restoration, not dramatic change.

Is Ellansé a filler or a collagen stimulator?

Ellansé sits in a category of its own. While it is technically classified as a dermal filler, it is more accurately described as a collagen biostimulator.

Unlike traditional hyaluronic acid fillers, which primarily add volume by attracting and holding water, Ellansé works by stimulating your body’s own collagen production. It provides some immediate structural support through its gel carrier, but its real benefit comes from the gradual production of new collagen over the months that follow treatment.

For this reason, many patients choose Ellansé when their goal is not simply to add volume, but to improve skin quality, facial support, and long-term skin health.

What is Polycaprolactone (PCL)?

Polycaprolactone, often shortened to PCL, is a biocompatible and biodegradable polymer that has been used safely in medicine for decades, including in dissolvable sutures and medical devices.

In Ellansé, tiny PCL microspheres act as a scaffold that stimulates the skin’s fibroblasts to produce new collagen. Over time, the PCL gradually breaks down and is naturally absorbed by the body, leaving behind the collagen that has been produced in response.

This regenerative process is what makes Ellansé different from many traditional fillers. Rather than simply replacing lost volume, it encourages the skin to rebuild its own structural support.

Can Ellansé replace traditional dermal fillers?

Sometimes, but not always.

Ellansé and hyaluronic acid fillers serve different purposes and are often used together rather than viewed as alternatives.

Ellansé is particularly effective for restoring structure, improving skin quality, and supporting collagen regeneration. Hyaluronic acid fillers remain valuable when precise shaping, contouring, or hydration is required.

At Mediject, treatment decisions are never based on a single product. Instead, we assess the whole face and recommend the most appropriate treatment, or combination of treatments, based on your anatomy, goals, and stage of ageing.

The best treatment plan is rarely about choosing one product over another. It is about selecting the right tool for the right purpose.

Is Ellansé suitable for younger patients?

Yes, in carefully selected patients.

While Ellansé is commonly used in patients experiencing collagen loss, it is increasingly being used as part of a collagen banking strategy in patients in their thirties and early forties.

Collagen production naturally begins to decline from our mid-twenties onwards. By supporting collagen production earlier, it may be possible to preserve skin quality and structural support for longer.

This does not mean everyone should start treatment young. The decision should always be based on an individual assessment rather than age alone.

At Mediject, we favour thoughtful intervention over unnecessary treatment. Sometimes the best treatment recommendation is no treatment at all.

Who should not have Ellansé?

Ellansé is not suitable for everyone.

Treatment may not be appropriate for patients who are pregnant or breastfeeding, have active skin infections in the treatment area, certain autoimmune or inflammatory conditions, known allergies to any of the product components, or unrealistic expectations about what treatment can achieve.

It may also not be the right choice for patients seeking immediate, dramatic change or those who are uncertain about a long-lasting regenerative treatment.

Because Ellansé cannot be dissolved in the same way as hyaluronic acid fillers, careful patient selection is essential.

This is why every treatment journey at Mediject begins with a detailed consultation. Understanding your goals, anatomy, medical history, and long-term treatment plan allows us to determine whether Ellansé is the right treatment for you, or whether another approach would be more appropriate.

What is collagen banking?

Collagen banking is the concept of preserving and building collagen reserves before significant collagen loss occurs.

Just as you might contribute to a pension before retirement, collagen banking involves investing in your skin’s future by supporting collagen production before visible ageing becomes pronounced.

The aim is not to stop ageing. It is to help the skin age more gradually by maintaining its structural support, elasticity, and resilience over time.

Treatments such as Ellansé, JULÄINE™, and other regenerative therapies can play a role in a long-term collagen banking strategy when used appropriately and as part of a personalised treatment plan.

What is Ellansé called in Chinese?

Ellansé is commonly known as 洢莲丝 amongst Mandarin-speaking patients. If you are researching collagen stimulators in Chinese-language publications, forums, or social media, you may see both Ellansé and 洢莲丝 used interchangeably. At Mediject Clinic in London, Dr. Sheila is well known for treating Asian faces regularly and understands the importance of providing clear information across different languages and cultures.

Ellansé is not a treatment for everyone. It is a treatment for the right patient, at the right time, delivered with the right plan. At Mediject, every recommendation begins with a comprehensive facial assessment, because exceptional results come from understanding the whole face, not simply treating an area.

If you are considering Ellansé and would like to explore whether it forms part of your long-term skin health and collagen banking strategy, we invite you to book a consultation at our Harley Street clinic.
