One of the questions we hear more frequently in clinic is: “I’ve been researching collagen stimulators and keep seeing JULÄINE™ and Sculptra. What’s the difference, and which is better?”
It is a great question. Both are designed to stimulate your body’s own collagen production rather than simply adding volume. Both have an important place within regenerative aesthetic medicine. However, while they belong to the same family of treatments, they are not the same product, and they are not always the right choice for the same patient.
Understanding those differences is not about choosing the newest treatment. It is about choosing the treatment that best supports your skin, your stage of ageing, and your long-term goals.
At Mediject Clinic, we do not believe there is a universally “best” collagen stimulator. We believe there is a treatment that is best suited to you.
Why collagen matters more than volume
From our mid-twenties onwards, collagen production begins to decline. Over time, this contributes to loss of firmness, reduced elasticity, skin laxity, fine lines, changes in facial support, and reduced skin resilience.
For many years, aesthetic medicine responded to these changes by replacing what had been lost. Adding volume. Filling lines. Correcting what time had changed.
Modern regenerative medicine takes a different approach. Rather than replacing volume, regenerative treatments aim to support the biological processes that maintain healthy tissue. This is why collagen stimulation has become such an important part of skin longevity and healthy ageing.
Both JULÄINE™ and Sculptra sit within this regenerative philosophy. But understanding how they differ is what allows us to make the right recommendation for each individual patient.
What JULÄINE™ and Sculptra have in common
Both belong to a class of treatments known as collagen biostimulators. Both contain poly-L-lactic acid, or PLLA. Both work by encouraging the body to produce new collagen over time rather than adding immediate volume. Both are designed to deliver gradual, natural-looking improvement. And both can form part of a wider collagen banking strategy.
This is where the similarities begin to diverge.
The question is not what the product contains, but how it is engineered
One of the most significant developments in biomaterial science over recent years has been the growing understanding that particle design influences biological behaviour. Shape matters. Surface matters. Uniformity matters. The way a biomaterial interacts with the immune system matters.
JULÄINE™ is built using proprietary LaSynPro™ technology, producing smooth, spherical PLLA microspheres with a highly uniform size distribution and a median size of approximately 32.6 microns. These microspheres are engineered to maintain their shape throughout degradation.
Research has demonstrated that smoother, spherical biomaterials generally provoke a lower tissue reaction than irregular or angular particles. Particle shape influences how immune cells interact with implanted materials and can affect inflammatory signalling pathways.
Sculptra uses PLLA microspheres produced through a different manufacturing process, with a different particle profile. It works by triggering a controlled inflammatory response that stimulates collagen synthesis over time.
Both mechanisms stimulate collagen. But the biological pathway through which they do so differs meaningfully.
Regeneration versus inflammation
Historically, many collagen stimulators have relied upon a degree of controlled inflammatory response. Inflammation can stimulate collagen production. However, inflammation is not always synonymous with regeneration.
As our understanding of ageing evolves, there is increasing recognition that chronic low-grade inflammation, often referred to as inflammageing, contributes to collagen breakdown, impaired cellular repair, and accelerated skin ageing.
Emerging research around JULÄINE™ suggests that its microsphere technology may promote a more regenerative cellular response characterised by fibroblast activation, extracellular matrix production, angiogenesis, and M2 macrophage polarisation. These are pathways typically associated with tissue repair and regeneration rather than inflammatory reaction.
The goal, as we see it at Mediject, is not simply collagen production. It is collagen regeneration with minimal unnecessary inflammation.
For a deeper understanding of the science behind JULÄINE™, our guide to what JULÄINE™ is and how it works covers this in detail.
How many sessions does each require?
This is often where the practical difference between the two treatments becomes most relevant for patients.
JULÄINE™ is typically delivered as a course of 2 sessions spaced 4 to 6 weeks apart, with maintenance recommended every 12 to 18 months depending on individual response. Many patients appreciate seeing some early improvement from the initial treatment while the longer-term regenerative process develops.
Sculptra traditionally requires a course of 3 or more sessions, with full results not visible until several months after the final session. Maintenance is then typically recommended annually.
For some patients, the more contained JULÄINE™ treatment journey is appealing. Others may be better suited to Sculptra depending on their specific clinical picture, skin quality, and goals. Neither is universally superior. The right choice depends on the individual.

Collagen banking and the bigger conversation
One of the biggest shifts we have seen over the past few years is the type of questions patients are asking. Rather than saying “I think I need filler,” they are asking “What can I do now to help my skin age well over the next ten years?”
That is a very different conversation. And it is exactly where collagen banking becomes so valuable.
Collagen banking involves supporting collagen production before significant collagen loss occurs. Rather than correcting ageing once it has become visible, the aim is to preserve tissue health proactively, investing in the skin’s future while it still has the reserves to respond robustly.
Both JULÄINE™ and Sculptra can contribute to a collagen banking strategy. JULÄINE™’s regenerative, low-inflammation mechanism aligns particularly closely with this philosophy, but the right choice depends on your age, skin quality, treatment history, and long-term goals.
It is also worth noting that collagen banking is not solely the domain of these two treatments. Other regenerative options including Ellansé, Radiesse, polynucleotides, and Sunekos all have a role to play depending on the individual patient’s needs. For patients specifically comparing JULÄINE™ and Radiesse, our JULÄINE™ vs Radiesse guide covers those differences in depth. At Mediject, we consider the full picture rather than defaulting to a single product.
The future of regenerative aesthetics
The future of aesthetics is unlikely to be defined by who can create the most volume. It will be defined by who can preserve tissue health most effectively.
Patients are increasingly seeking natural outcomes, skin quality improvement, healthy ageing, and long-term skin resilience. Not all collagen stimulators are the same. Not all PLLA technologies are the same. The way a material is engineered influences how tissue responds.
Why choosing the right clinician matters more than choosing the right collagen stimulator
Patients often spend hours comparing products online. In reality, the product is only one part of the equation.
The quality of your consultation, the assessment of your facial anatomy, an understanding of how your skin is ageing, and a carefully considered long-term treatment plan will have a far greater impact on your outcome than the brand name on the syringe.
At Mediject Clinic, we do not begin by asking which collagen stimulator you would like. We begin by asking: What are you hoping to achieve? What concerns you most? How do you want to age? What matters most to you?
Sometimes JULÄINE™ is the right answer. Sometimes Sculptra is. Sometimes another regenerative treatment may be more appropriate. And occasionally, the right recommendation is not to treat at all.
That philosophy is one of the reasons many of our patients stay with us for years.
Dr Sheila’s perspective
Dr Sheila did not introduce JULÄINE™ into Mediject because it was new. She introduced it because it aligned with a philosophy she has held throughout her career. That aesthetic medicine should support the body’s biology, not simply replace what time has changed.
As one of the earliest clinicians in the UK to introduce JULÄINE™, and now a UK faculty member and national trainer, she has seen first-hand how regenerative treatments are changing both patient expectations and clinical practice.
For Dr Sheila, this is not about replacing one collagen stimulator with another. It is about helping patients move away from chasing volume and towards preserving healthy skin for the future.
Frequently asked questions
Which lasts longer, JULÄINE™ or Sculptra?
Both produce long-lasting results through genuine collagen stimulation. JULÄINE™ results typically last between 12 and 18 months following a course of 2 sessions. Sculptra results can last up to 2 years following a full course, though both treatments require periodic maintenance to sustain results over time.
Is JULÄINE™ better than Sculptra?
Neither treatment is universally better. Both stimulate collagen through PLLA technology, but JULÄINE™ is engineered with a focus on regenerative biology rather than inflammatory stimulation, which may offer a more predictable, lower-inflammation treatment experience. The right choice depends on individual skin quality, treatment goals, and clinical assessment.
Can JULÄINE™ and Sculptra be used together?
In some cases, yes. A clinician may recommend different biostimulators for different areas of the face, or sequence them over time within a broader regenerative plan. This would always be assessed carefully at consultation.
Which treatment is better for collagen banking?
Both can contribute to a collagen banking strategy. JULÄINE™’s regenerative, low-inflammation mechanism aligns particularly closely with this philosophy, but the right choice depends on your age, skin quality, and long-term goals. Other treatments including Ellansé and polynucleotides may also form part of a broader strategy.
Is JULÄINE™ painful compared to Sculptra?
Both treatments are generally well tolerated. JULÄINE™ has been designed to flow more easily through smaller needles and require less injection force, which may contribute to a more comfortable treatment experience, though individual sensitivity varies.
How do I know which treatment is right for me?
The right treatment is rarely determined by a product alone. It is determined by your anatomy, your stage of ageing, your skin quality, and your long-term goals. At Mediject, every recommendation begins with a comprehensive consultation because exceptional results come from understanding the whole face, not simply choosing a treatment.
Whether JULÄINE™, Sculptra, Ellansé, Radiesse, or another regenerative treatment is appropriate, the decision should never begin with a product. It should begin with understanding you. Your skin. Your facial anatomy. Your stage of ageing. Your concerns. And most importantly, how you want to look in five, ten, and twenty years’ time.
At Mediject, that is where every consultation begins.
Because exceptional outcomes are rarely created by choosing the newest treatment. They are created through thoughtful planning, clinical expertise, and a philosophy built around helping you age well while continuing to recognise yourself in the mirror.
