One of the most common things I hear in clinic is: “I’m happy with how I look. I just don’t want to lose myself.”
Patients rarely ask me how they can look twenty years younger. They ask something much more thoughtful.
“How do I age well?” “How do I maintain what I have?” “How do I avoid looking overdone?”
Those questions have changed aesthetic medicine. They have certainly changed mine.
This shift has driven one of the most significant developments in aesthetic medicine in recent years: the rise of regenerative aesthetics. JULÄINE™ (艾维岚) sits at the centre of that conversation.
Why Dr Sheila uses JULÄINE™
I was among the earliest clinicians in the UK to introduce JULÄINE™ into clinical practice. I now train other doctors in its safe, evidence-based use, hold a UK faculty position with the JULÄINE™ team, and lecture on regenerative medicine at the Aesthetic Medicine MSc at Queen Mary University of London, my own alma mater.
I do not use JULÄINE™ because it is new or because it is generating interest. I use it because the science behind it is genuinely compelling, and because it represents the kind of thoughtful, biology-led approach to ageing that I have always believed in.
The shift JULÄINE™ represents, from inflammatory biostimulation toward regenerative biomaterial design, aligns directly with how I have always practised. I am not interested in forcing the skin to respond. I am interested in supporting the skin’s own ability to regenerate.
That distinction matters enormously. And it is why, when I recommend JULÄINE™ at Mediject, it is never a quick decision. Care begins long before the first injection.
Dr Sheila holds a Master’s degree in Aesthetic Medicine with distinction from Queen Mary University of London. She has treated over 19,000 patients across more than 11 years of clinical practice and has personally trained over 900 clinicians in advanced facial aesthetics through Mediject Academy.
What JULÄINE™ is, and what it is not
JULÄINE™ is a collagen biostimulator. More specifically, it is a next-generation poly-L-lactic acid (PLLA) injectable designed around advances in biomaterial science, particle engineering, and regenerative biology.
It is not a filler. It does not add volume, and it does not produce an immediate visible change in the way that hyaluronic acid fillers do. Its purpose is not to replace what has been lost. Its purpose is to stimulate the skin’s own biological processes, encouraging the production of new collagen, supporting fibroblast activity, and contributing to the long-term health and structural integrity of the skin.
This distinction matters. Most of what patients encounter in aesthetic clinics is still built around the replacement model. Hyaluronic acid fillers, which dominate the market, work by introducing a substance that holds space within the tissue until the body breaks it down. The result is immediate but temporary, and the process needs repeating.
JULÄINE™ takes a fundamentally different approach. Rather than introducing something from outside, it activates something from within.
Why collagen matters more than volume
To understand why JULÄINE™ represents such a significant step forward, it helps to understand what actually happens to the skin as it ages.
From our mid-twenties onwards, collagen production begins to decline by approximately 1% per year. In women, this decline accelerates significantly during the menopausal transition, when falling oestrogen levels directly affect collagen synthesis. Over time, the effects accumulate.
Skin firmness decreases. Elasticity reduces. Facial support softens. The structural scaffolding that gives the face its definition, lift, and resilience gradually weakens.
Historically, aesthetic medicine responded to these changes by replacing volume. Whilst volume replacement has its place, it does not address the underlying biological process. It treats the appearance of collagen loss without supporting collagen itself.
Regenerative aesthetics takes a different view. Rather than masking the changes that come with age, it aims to support the biological mechanisms responsible for maintaining healthy tissue. The goal is not correction. It is preservation, regeneration, and long-term skin health.
The science behind JULÄINE™
JULÄINE™ is built using proprietary LaSynPro™ technology. This produces smooth, spherical PLLA microspheres with a highly uniform size distribution. The microspheres have a median size of approximately 32.6 microns and are engineered to maintain their shape throughout the degradation process.
To understand why this matters, it helps to understand what fibroblasts are and why they are so important.
Fibroblasts are among the most critical cells in skin health and ageing. They are responsible for producing collagen, both Type I which provides structural strength, and Type III which supports tissue repair and elasticity. They also produce elastin, hyaluronic acid, and the extracellular matrix proteins that form the broader architecture of healthy skin.
As we age, fibroblast activity declines. This is one of the fundamental drivers of visible ageing. Supporting fibroblast function is therefore not simply about stimulating collagen production. It is about restoring one of the skin’s most important regenerative processes.
When JULÄINE™ microspheres are injected into the skin, they interact with the surrounding tissue in a way that promotes fibroblast activation, extracellular matrix production, and angiogenesis, the development of new blood vessels that support tissue health. Research presented for JULÄINE™ suggests enhanced fibroblast migration and extracellular matrix activity when compared with traditional PLLA technologies.
Regeneration versus inflammation
One of the most important distinctions in modern regenerative medicine is the difference between regeneration and inflammation.
Historically, many collagen stimulators have been described as working through a controlled inflammatory response. Inflammation can stimulate collagen production, but inflammation is not the same as regeneration, and the two should not be treated as equivalent.
As our understanding of ageing evolves, there is increasing recognition that chronic low-grade inflammation, often referred to as inflammageing, contributes to collagen breakdown, impaired cellular repair, and accelerated skin ageing. This raises an important question about treatments that rely on an inflammatory mechanism to produce their results.
The engineering behind JULÄINE™ is designed with this in mind. The smooth, spherical morphology of LaSynPro™ microspheres is intended to promote a more regenerative cellular response, characterised by the kinds of biological pathways associated with tissue repair rather than inflammatory reaction. The goal is not simply collagen production. It is collagen regeneration with minimal unnecessary inflammation.
This distinction is at the heart of why JULÄINE™ represents a genuinely different approach to collagen stimulation.
Why less is often more
One of the biggest misconceptions in aesthetic medicine is that better results come from doing more.
In reality, the opposite is often true.
At Mediject Clinic, one of the most common things patients say during their consultation is: “I’m happy with how I look. I just want to maintain it. I don’t want to go too far.” That conversation is at the heart of everything we do. Our patients are not trying to become someone different. They are trying to preserve what already makes them look like themselves.
That is why our treatments are rarely designed around a single appointment. Instead, we develop a personalised, long-term plan that allows results to evolve gradually and naturally over time. Sometimes that means recommending less treatment than a patient expected. Sometimes it means delaying treatment altogether. Sometimes it means saying, “I don’t think you need anything else today.”
Those conversations are just as important as the treatments themselves. Care begins long before the first injection.
In aesthetic medicine, restraint is not the absence of skill. It is often the greatest demonstration of it. When treatments are carefully staged, the face has time to adapt, collagen has time to regenerate, and each decision can be made with greater confidence. Rather than chasing immediate change, we focus on preserving facial harmony and supporting healthy ageing over the years ahead.
Our goal has never been for someone to notice your treatment. Our goal is for someone to notice you.
The best compliment should never be: “Who did your filler?” It should be: “You look well.” “You look rested.” “There’s something different about you. I just can’t quite put my finger on it.”
That is the difference between changing a face and caring for one. It is also why regenerative treatments such as JULÄINE™ fit so naturally within our philosophy. They do not replace what makes you unique. They work with your own biology to support healthier skin, stronger collagen, and natural ageing over time.
Because ageing well is not about looking younger than everyone else. It is about continuing to recognise yourself every time you look in the mirror.
What is the extracellular matrix and why does it matter?
Youthful, resilient skin is not simply about collagen levels. It is about the health of the extracellular matrix, the complex structural environment in which skin cells live and function.
The extracellular matrix includes collagen, elastin, glycosaminoglycans such as hyaluronic acid, and a range of proteins and signalling molecules that support cellular communication, vascular health, and tissue integrity. As we age, the quality of the extracellular matrix declines alongside collagen production. Skin becomes not just less firm, but less functional.
Treatments that support fibroblast activity and extracellular matrix health address this broader picture of skin ageing. Rather than simply adding structure from outside, they work to restore the internal environment that healthy skin depends on.
How JULÄINE™ fits into a collagen banking strategy
At Mediject, many patients are no longer asking how to look younger. They are asking how to maintain healthier skin for longer.
This is the philosophy behind collagen banking.
Much like contributing to a pension before retirement, collagen banking involves supporting collagen production before significant collagen loss occurs. Rather than correcting ageing once it has become visible, the aim is to preserve tissue health proactively, investing in the skin’s future while it still has the reserves to respond robustly.
JULÄINE™ aligns particularly closely with this approach. Because it works by stimulating the skin’s own biological processes rather than replacing lost volume, it contributes to the collagen reserve rather than simply masking its absence.
For patients in their thirties and early forties, early collagen stimulation can make a meaningful difference to how the skin ages over the following decade. For patients in their forties and beyond, JULÄINE™ can form part of a longer-term strategy to maintain skin quality, structural support, and resilience as the body’s natural collagen production continues to decline.
JULÄINE™ and skin longevity
Skin longevity is where the aesthetics industry is heading. Not anti-ageing. Not looking younger. Maintaining healthy, functional skin for as long as possible.
This language is more sophisticated, more medically credible, and more aligned with how patients are increasingly thinking about their skin. They are not seeking transformation. They are seeking preservation.
JULÄINE™ supports skin longevity in several ways. By activating fibroblasts, it supports the ongoing production of the structural proteins the skin needs to maintain its quality. By promoting extracellular matrix health, it supports the broader biological environment in which healthy skin functions. And by contributing to collagen reserves rather than depleting them, it works with the natural ageing process rather than against it.
This is what distinguishes regenerative aesthetics from conventional aesthetic treatments. It is not about fighting ageing. It is about ageing well.
What JULÄINE™ treatment involves
A typical JULÄINE™ treatment at Mediject takes approximately 45 to 60 minutes. Unlike hyaluronic acid fillers, which produce an immediate visible change, JULÄINE™ results develop gradually over the weeks and months following treatment as collagen production builds and the skin responds to the biostimulant.
Most patients notice an improvement in skin quality, firmness, and luminosity within 6 to 8 weeks. Full results are typically visible at around the 12 week mark, with continued improvement beyond that point as collagen remodelling progresses.
Downtime is minimal. Some temporary swelling, tenderness, or redness around the injection sites is normal and settles within a few days. Most patients return to normal activities the same day.
Unlike hyaluronic acid fillers, JULÄINE™ cannot be dissolved after injection. This makes the quality of the clinical assessment and the experience of the treating clinician particularly important before treatment begins. Care begins long before the first injection.
Who is JULÄINE™ for?
At Mediject Clinic, we do not begin by deciding whether JULÄINE™ is the right treatment. We begin by understanding whether treatment is needed at all.
JULÄINE™ is suitable for patients who are primarily interested in skin quality, collagen support, and long-term skin health rather than immediate volume correction.
It is particularly well suited to patients who are noticing gradual changes in skin firmness, texture, and luminosity. Patients who want to take a proactive approach to ageing through collagen banking. Patients in perimenopause or menopause experiencing accelerated collagen decline. Patients who are cautious about volumising treatments and prefer a more biological approach. Patients who have had previous aesthetic treatments and are looking to transition toward a more regenerative strategy.
It is not used in the lips, tear troughs, or nose, and it is not appropriate for every patient. Suitability is always assessed at consultation through a full-face assessment that considers anatomy, skin quality, stage of ageing, and long-term goals.
For patients weighing up JULÄINE™ against other biostimulators, our guides to JULÄINE™ vs Sculptra and JULÄINE™ vs Radiesse explore those comparisons in detail.
Why patients who choose JULÄINE™ often are not looking for filler at all
Many of the patients who come to Mediject asking about JULÄINE™ are not really asking about a treatment. They are asking something deeper.
They have seen what happens when aesthetics goes too far. A friend whose face looks pillow-like. A colleague who suddenly looks like a different person. A partner who came home from a clinic looking overdone, and who now regrets it. These patients are not anti-aesthetics. They are anti-risk. They want to maintain themselves. They want to age well. They want to remain completely believable.
For many of them, JULÄINE™ is not a treatment decision. It is the start of a conversation.
A conversation about how their skin is ageing. What is changing. What they want to preserve. And how we might be able to help them continue recognising themselves for years to come.
That is exactly the kind of consultation we are built for at Mediject.
Frequently asked questions
What is JULÄINE™ and how is it different from other collagen stimulators?
JULÄINE™ is a next-generation poly-L-lactic acid collagen biostimulator built using LaSynPro™ microsphere technology. Unlike traditional PLLA treatments that rely primarily on an inflammatory response to stimulate collagen, JULÄINE™ is engineered to promote a more regenerative cellular response, including fibroblast activation, extracellular matrix support, and angiogenesis. It is designed for collagen regeneration with minimal unnecessary inflammation.
Is JULÄINE™ a filler?
No. JULÄINE™ is a collagen biostimulator, not a filler. It does not add volume and does not produce an immediate visible change. Its purpose is to stimulate the skin’s own collagen production and support long-term skin health. Patients seeking immediate volumisation are better suited to hyaluronic acid fillers, which serve a different purpose.
How long does JULÄINE™ take to work?
Results develop gradually. Most patients notice an improvement in skin quality and firmness within 6 to 8 weeks. Full results are typically visible at around the 12 week mark, with ongoing improvement as collagen remodelling continues. The process is gradual by design. This is a regenerative treatment, not an instant result.
How long do JULÄINE™ results last?
Results typically last between 12 and 18 months depending on individual factors including metabolism, skin quality, lifestyle, and sun exposure. Because JULÄINE™ stimulates real collagen production, the improvements it creates are genuine and structural rather than simply filling space.
Can JULÄINE™ be used as part of a collagen banking strategy?
Yes. JULÄINE™ is particularly well suited to collagen banking because it works by stimulating the skin’s own biological processes rather than replacing lost volume. For patients in their thirties and forties who want to support their collagen reserves before significant loss has occurred, it can form a valuable part of a longer-term skin health strategy.
How many sessions of JULÄINE™ will I need?
Most patients begin with a course of 2 sessions spaced 4 to 6 weeks apart. Maintenance sessions are then typically recommended every 12 to 18 months depending on individual response and goals. The exact plan is always determined at consultation based on a thorough assessment.
Is JULÄINE™ suitable for Asian skin?
Yes. JULÄINE™ is well suited to Asian patients and forms part of Mediject’s approach to culturally sensitive, anatomy-led treatment planning. Asian patients often retain facial volume for longer than Caucasian patients but may experience different patterns of collagen decline, skin quality change, and structural ageing. Treatment planning at Mediject takes ethnicity, facial proportions, and facial identity into account. Dr Jiun brings specialist expertise in Chinese and East Asian aesthetics within the Mediject team.
Is JULÄINE™ safe?
Yes. JULÄINE™ has a strong safety profile and is delivered at Mediject by medically qualified clinicians with extensive training in regenerative injectables. As with all injectable treatments, there are potential side effects including temporary swelling, bruising, and tenderness. Unlike hyaluronic acid fillers, JULÄINE™ cannot be dissolved after injection, which makes thorough consultation and careful patient selection particularly important.
What is the difference between JULÄINE™ and Profhilo?
JULÄINE™ and Profhilo serve different purposes. Profhilo is a bio-remodelling treatment that primarily delivers intense hydration and supports skin quality through high-concentration hyaluronic acid. JULÄINE™ is a collagen biostimulator that works by activating fibroblasts and stimulating collagen production over time. Both can complement each other within a broader skin health strategy, but they address different aspects of skin ageing. We explore this further in detail in our dedicated guide.
Can JULÄINE™ be combined with other treatments?
Yes. At Mediject, JULÄINE™ is often used as part of a wider treatment plan that may include Ellansé for structural collagen stimulation, botulinum toxin to reduce muscle pull and protect results, or skin health treatments to support the skin’s surface quality alongside the deeper regenerative work. All combinations are planned carefully and introduced gradually. The goal is always a considered, staged approach rather than doing everything at once.
JULÄINE™ is not the right treatment for everyone. And that is exactly why every journey at Mediject Clinic begins with a conversation, not a syringe.
Sometimes the best recommendation is JULÄINE™. Sometimes it is another regenerative treatment. Sometimes it is simply good skincare, time, and a plan.
Because our job is not to sell treatments. It is to help you understand how your face is ageing, what your skin needs, and how we can help you continue recognising yourself for years to come.
That is the philosophy behind every consultation at Mediject Clinic.
